We would like you to answer our survey: Los Angeles City/County Native American Indian Commission's (LANAIC) Self Governance Board (SGB) is doing this survey to learn about the social service needs of our American Indian and Alaska Native community. If your agency serves Los Angeles County and sees American Indian and Alaska Native clients, we want to know about available resources and services to meet those needs and improve self-sufficiency of our relatives living in poverty.

Voluntary participation: Your participation in this survey is completely voluntary. You may choose not to answer any questions. You may stop the survey at any time. There is no penalty or loss of benefits to you if you choose to do so.

What is being asked of you: If you decide to participate in this survey, we will ask you about your agency, what services you provide for American Indian and Alaska Native clients, information on service needs and availability, and barriers to access. You may skip or refuse to answer any questions on the survey. The survey will take around 10-15 minutes.

Benefits to you: Your answers help local American Indian and Alaska Native organizations identify service gaps and ensure that services meet the needs of our relatives living in poverty.

Possible risks: Some of the questions may make you feel uncomfortable or sad. Again, you can choose to skip any questions that you wish.

Personal Information: Your responses will be kept private and anonymous. We will not ask for your name, title, street address, or telephone number. All responses will be reported only as combined information and will not identify you or your agency specifically.
