Please use this form to report concerns about COVID-19 in healthcare facilities and other workplaces.

Healthcare and social service settings, including acute care facilities, urgent care, long-term care facilities, senior living communities, outpatient clinics, supportive or transitional housing, homeless shelters, dialysis centers, and correctional facilities, may use this form to report or update cases among staff and clients.

Other workplaces may use this form to voluntarily report concerns of COVID-19 transmission in non-healthcare business settings. This includes reporting suspected workplace transmission or outbreaks, or requesting technical assistance for protecting workers or clients from COVID-19 transmission. Non-healthcare businesses are not required to report individual cases of COVID-19.

To report concerns regarding compliance to state or public health orders, please call the King County COVID-19 Business and Community Information Line (Monday-Friday) 8:30 AM – 4:30 PM at 206-296-1608.

The Public Health - Seattle & King County COVID-19 Investigation Team will review all submitted forms and follow-up as needed.
