Community Health Workers at Public Health - Seattle & King County have been supporting families with asthma care in King County for over 20 years. The program has helped children and adults have more days without asthma symptoms, fewer emergency room visits, fewer missed days at school or work, and better quality of life. The program consists of virtual home visits, education for patients with asthma and their families, and equipping them with the tools they need to improve their health. Clients/patients can be referred to our program by clinics, schools, or self-report by completing this survey.

 

Clients / patients referred to this program must:

• Consent to home, video, or telephone visits

• Have poorly controlled, moderate or severe persistent asthma

• Be a resident of King County

• Be at least 5 years old (younger children considered on a case-by-case basis)
