Anchorage Foundations Exam Reschedule Attestation Form

Please consult the Foundations Phase Exam Reschedule Policy before completing this form.

UWSOM Testing Service and your Foundations Dean/Admin will have access to the information you submit on this form.

If you are requesting a reschedule for personal illness or personal/family emergency, this form must be submitted before the start of the exam (or as soon as possible if there are extenuating circumstances). False attestation of personal illness or personal/family emergency is considered a violation of the UWSOM Standards of Conduct and Professional Behavior.

If you are requesting a reschedule for a personal event, this form must be submitted at least two weeks prior to the exam date. Students who are not on academic probation are permitted to reschedule one exam per term for personal events. Pin tests and NBME exams (e.g., CAS, CBSE) cannot be rescheduled for personal events.
